How to Use This Thought Record

When you notice a strong negative emotion, work through the 7 columns left to right:

  1. Situation — What happened? Just the facts, objectively.
  2. Emotions — What did you feel? Rate intensity 0–100%.
  3. Automatic thoughts — What went through your mind? What's the hot thought?
  4. Cognitive distortions — Which thinking traps are you using? (See reference table below.)
  5. Evidence for — What facts support the hot thought?
  6. Evidence against — What facts contradict it?
  7. Balanced thought — A fairer, more accurate alternative. Re-rate emotion intensity.

Cognitive Distortions Reference (Column 4)

DistortionWhat it looks like
All-or-nothing"If I'm not perfect, I'm a total failure."
Overgeneralization"This always happens to me. I never get it right."
Mental filterFocusing only on the one negative thing, ignoring positives.
Disqualifying positive"That doesn't count because..."
Mind-reading"They probably think I'm stupid."
Fortune-telling"This is going to be a disaster."
Catastrophizing"If I fail this, my whole career is over."
Emotional reasoning"I feel guilty, so I must have done something wrong."
Should statements"I should be over this by now."
Labeling"I'm an idiot." (vs. "I made a mistake.")
Personalization"It's my fault they're in a bad mood."
